The In Reach Team works across East Berkshire assisting in the safe and timely discharge of patients to the community from the acute sector. The In Reach Coordinator will work alongside the In Reach team and the acute sector to provide coordinated care for adults requiring discharge, reducing the length of stay for patients in the acute setting.

The post holder will be the primary point of contact for all discharges from the acute sector into community beds or nursing teams.

The post holder will represent BHFT on the daily hospital conference calls, updating on the capacity of the community beds and nursing teams.

The In Reach Coordinator will assist in facilitating a discharge from secondary care, working closely with the In Reach Team, Intermediate Care, Community Health services, Primary Care and Mental Health services to ensure robust proactive care planning. The In Reach Coordinator will be a member of the Integrated Team and will be the point of contact for the In Reach Team and other professionals within the Community, local acute and community hospitals to manage the safe transfer of patients from the Emergency Department and wards back into the community, including patients' homes, nursing and residential homes and community beds.

The In-reach team is a small team that has developed over the last 8 years we have built up very strong working relationships with both the acute and Community services.
