An opportunity has arisen for a passionate and sympathetic individual to join our team as a Seibiant Support Worker based in Swansea.

**Contract**:37.5 hours per week (Monday - Friday), permanent

**Salary**: £21,507.75 per annum

**Located at**: Uplands, Swansea

**Responsible to**:Service Manager

**About the Job**:
Adferiad Seibiant Service provides support to individuals who are experiencing or recovering from a serious mental illness. These support sessions are to enable the carer, or family member of the individual to have respite/break from their caring role. The role of the Seibiant Support Worker is to assess referrals and organise support for each individual in their community to increase their own living skills and confidence and develop links with other local, community-based resources and opportunities. The post holder would also provide relevant information to the carer so that they are more informed of the rights and opportunities within the City and County of Swansea.

**Responsibilities**:

- To provide direct support to individuals with varying levels of mental health need/diagnosis.
- To work with and assist individuals to transition from secondary care back into the community.
- Adopt a strengths-based, recovery focused approach, which facilitates an environment where individuals can lead their own recovery.
- Develop and maintain clear boundaries and establish clear lines of communication.
- Work in partnership with the Service Manager to ensure that monitoring and reporting objectives are met, data is reviewed, and any changes required are undertaken promptly and without disruption to service delivery.
- Establish a good understanding of the community and the importance of networking, developing robust professional relationships with a variety of partners/services within your area of work.
- Support clients to access appropriate services and resources as identified in the personal plan, care and treatment plan and/ or as required.
- Using a strength-based approach to encourage and support service users to develop their own transition plan which highlights the activities and services they would like to engage with in their community after discharge, and the goals they would like to achieve.
- Work effectively with the Occupational Therapists and other multi-disciplinary teams in supporting the Recovery through Activity programme.
- Providing one-to-one support sessions.
- To enable clients to exercise informed choices about their lives where possible and to enhance their social skills and participate in the community.
- Supporting individuals through a range of activities, on an on-site and peripatetic basis.
- Promote, encourage and facilitate the use of local community services.
- Identify and meet the needs of individuals to enable them to maintain their wellbeing.
- Provide information, advice and guidance as appropriate and in a timely manner.
- Assist individuals in identifying areas of support and identifying the best option of support in line with their Care & Treatment Plan.
- Complete individual support/recovery plans/exit plans and other documentation as required.
- Signposting and referring carer/wider family members for additional support where appropriate.
- Contribute to monitoring and evaluation processes to ensure the service is effective, accountable and able to promptly identify concerns, taking advice and action to swiftly address them.
