We are looking for an energetic individual to fulfil the **Care Coordinator** role in **Coventry**, to oversee care delivery across **Coventry & Warwickshire**.
This position requires you to coordinate the day to day shift pattern and allocate work to Care Workers. You will also have the responsibility of the on-call phone, on a rota basis.
Duties include, but are not limited to:
- To allocate work to Care Workers, cover any unallocated work and distribute rotas;
- Manage requests for time off;
- To work in cooperation with all local authorities to ensure the safeguarding and wellbeing of Service Users are upheld at all times;
- Undertake the duties of the on-call phone when required, ensuring a detailed handover report is submitted;
- Document and report any issues to line manager;
- To attend team meetings and support new staff members;
- Performs all tasks in compliance with the company policies/rules and procedures;
- To prioritise and manage time effectively;
- To keep up to date with all mandatory training;
- To communicate with confidence to the service user/coworkers and record information clearly;
- To be able to work independently and as part of a team;
- Visiting Service Users, checking on the care they've received and documenting it accordingly.
