**Criminal Justice Continuity of Care Volunteer**

**About the project**
Forward Leeds is the alcohol and drug service in Leeds for adults, young people, and families. Forward Leeds is delivered by 4 services in partnership, led by Humankind.
- This exciting volunteer role would best suit someone with an empathetic, non-judgmental attitude who can build strong working relationships with others. This is a fantastic opportunity to get hands on experience in a drug and alcohol service and put your passion for supporting and advocating for vulnerable people to great use.
- This role is based within the criminal justice team who work with individuals to manage the transition from prison to community for those whilst in prison engaging with drugs team or on prescription. We are the service that offers the treatment part mandated court orders (drug treatment requirement/alcohol treatment requirement) in law that they have to attend certain groups/treatment. This is jointly managed with probation.**About the role**
The main aim of this role is to increase the number of people leaving prison moving into structured treatment for their substance/alcohol support.
- The role will also be to provide support for service users at the beginning of their time with Forward Leeds, encouraging them to access the necessary support.**Tasks will include**:

- Contact prisons to gather more information on a leaver ahead of their assessment, e.g. who the probation officer is.
- Follow up with leavers who miss their initial assessments, support them to attend within 21 days
- Chasing up confirmation of medication, to support organising prescriptions ahead of prison release.
- Liaising with the prison healthcare team, probation, clinical and drug and alcohol teams.
- To offer information, support and guidance to people accessing the hubs
- To liaise between the service user and reception
- Actively encourage service users into Forward Leeds
- To provide information to service users on Forward Leeds services and local services
- To support and promote a safe and recovery focused environment
- To actively listen to service users
- Invited to attend team meetings
- To promote and engage in service user influence and involvement

- Experience working with people who have experienced trauma
- Experience of working / volunteering with adults at risk / with support needs related to accommodation and / or drug/alcohol use

We ask that you can commit to at least half a day for a minimum of 6 months. The Forward Leeds service is open from Monday-Friday 9am-5pm.**Our workplace values are**:
