This is an exciting opportunity for an existing Health Coach to gain experience in a,, Senior Health Coach role. The role will be based in the public health team and is a 12 month fixed term post.

The senior health coach role is multi-skilled and will involve working across a variety of disciplines within health care.

The role will work alongside the public health team to contribute to the work to tackle inequalities in health and access to services.

This post will work into the existing maternity pathways, based primarily in an antenatal clinic setting, supporting and embedding the role of health coaches in maternity/obstetric care.

This role will provide individualised care within maternity services, working to promote a healthier lifestyle during pregnancy, including support with physical activity, healthy weight, substance misuse support, smoking cessation and pre term birth prevention.

The post holder will support the work of the health coach team, improving health, preventing ill-health and reducing morbidity associated with lifestyle, by supporting people to develop healthier behaviours, especially those at risk of poor health and with long term conditions, ensuring that delivery of the service is to agreed quality standards.

Supporting health coaches in acute settings to ensure successful delivery of the health coach service by providing day to day operational support.

Supporting and encouraging inpatients and outpatients to improve their health, wellbeing and lifestyle through motivational support and action planning, emphasis on self-care that empowers individuals to live well and stay well.

Helping patients to find and use the right services that supports addressing lifestyle issues that may be impacting on patient’s health and wellbeing.

Identifying new referral pathways and strengthening existing ones into the health coach service.

Function as a senior health coach within the clinical pathway to embed the health coach ethos/approach within the pathway

To support health coach manager in the coordination and organisation of the health coach service and to act as a point of contact for health coaches for issues pertaining to the practicalities of the day to day operational matters within the team

To support in the cascade of MECC Train the Trainer where applicable and seek to work with pathways and services to embed and evaluate some of the key MECC learning within their area

- Health and Wellbeing Coaches support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills,confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health related behaviours. Health coaches do this by coaching and motivating people through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support
- Health and Wellbeing Coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a person’s needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations. Health and Wellbeing Coaches may work with people by phone, by video conference, or face-to-face
