**Job summary**

We are seeking Healthcare Assistants to work within the community as part of our Neighbourhood Team. We provide care and treatments for housebound patients across Oxfordshire. This is a diverse role which requires you to have your own clinical list.
- We require clinical staff to be part of a multi-disciplinary clinical team working remotely with patients in their own homes.
- The service will operate 7 days a week, 8am-10pm.
- Ideally, you will have some experience in delivering health care. You will have an opportunity to work flexibly to fit in with personal circumstances.

**The purpose of the role is to**:

- Provide care and treatments for patients in their own homes.
- Work with the registered clinicians to complete tasks and provide care.
- Supported by the clinical team and appropriate training you will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, in the monitoring of long term condition, and health prevention and screening activities.

**Job responsibilities**

**Clinical skills**
- Undertake routine tasks related to individuals health and well being.
- Assist in the delivery of care to meet individuals health and wellbeing needs
- Establish and maintain communication with people on routine care matters.
- Assist in maintaining own and others health and safety and security.
- Acknowledge the risk of Healthcare Associated Infections (HCAI) and understand own responsibility as agreed with line manager in the prevention and control of HCAI.
- To maintain standards of infection control and ensure correct infection control procedures are followed when cleaning equipment.
- To meet PML values, whilst delivering care to patients and relatives.
- Complete clinical tasks such as, phlebotomy, processing of samples of e.g. urine, ECGs, blood pressure monitoring and simple dressings.
- Refer to trained staff when appropriate
- Carry out any other duty that reasonably falls within the general nature and level of responsibility of the post.
- Ensure all concerns are escalated in line with best practice.

**Governance**
- Assist with the management of referrals and allocation of work
- Ensure own actions help to maintain quality.
- Ensure own actions support equality, diversity and rights
- Provide and receive patient information, ensuring that all patient information is provided, received and recorded in accordance with the Data Protection Act and PML Policies.
- To ensure that Clinical Governance and risk management are embedded within daily practice.
- Responsible for stock taking, ensuring adequate supply and correct storage.
- Ensure all patient information is accessible and up to date
- To attend team meetings when requested whether virtual or face to face

**Personal Development**
- Contribute to own personal development.
- Ensure all patient information is accessible and up to date
- Attend training sessions and complete competencies to ensure own development and development of the service.

**Job Types**: Full-time, Part-time

**Salary**: £22,400.00-£23,000.00 per year

Expected hours: No more than 37.5 per week
