Role:
We are seeking a dedicated Care Coordinator to join our team at True Care 247. In this pivotal role, you will oversee the seamless delivery of high-quality care services, ensuring that our clients receive tailored support that meets their individual needs and preferences.

Key Responsibilities:
Efficiently manage care worker schedules, ensuring optimal matching of skills and availability with client requirements, while considering travel time, holiday cover, and any unforeseen changes.

Provide valuable insights to enhance operational practices, contributing to the continuous improvement of our care services.

Take the lead in sourcing new care packages and enhancing existing ones, fostering strong relationships with clients and their families.

Ensure continuity of care by meticulously coordinating care assignments for both new and existing clients.

Maintain accurate digital records for clients and care workers, utilising our bespoke system to uphold the highest standards of professionalism and accountability.

Handle customer inquiries promptly and courteously, safeguarding our company's reputation for excellence in care.

Ensure compliance with all relevant regulations and company policies, including those set forth by regulatory bodies such as the Care Quality Commission (CQC).

Support the development and engagement of our care worker community, facilitating training and performance evaluations as needed.

Contribute to the growth and expansion of our services in line with organisational objectives.

**Requirements**:
Preferably possess an NVQ/QCF Level 3 in Care or equivalent qualification.

Demonstrated experience of at least 2 years in a care coordination role within a domiciliary care agency.

Thorough understanding of CQC compliance requirements and experience in conducting care assessments and reviews.

Proven ability to motivate and support care staff, coupled with exceptional organizational skills and attention to detail.

Adaptability to thrive in a dynamic, fast-paced environment, with a willingness to embrace evolving roles and responsibilities.

Strong team player with excellent communication and interpersonal skills.
