**Hours**: 22.5 hours per week 3 days 9am - 5pm
Our client, a well-respected charity, is looking for a person to support people in a trauma-informed way to ensure they receive the right emotional and practical support and advice and assistance with their hoarding behaviour.
**Synopsis of duties**:
- Taking a hands-on approach to decluttering and making people’s homes safer whilst taking the time to get to know the person and help build their confidence and independent living skills, to support them to keep their tenancy
- Work with people to understand their needs to deliver a comprehensive and holistic package of support to help them to improve their quality of life, develop their skills, and sustain their tenancy.
- Ensure support is delivered with a trauma informed approach and people are fully consulted and involved in the planning and revision of their support plans and that they are relevant to the current preferences and situation of each person.
- Act as an individual ‘Keyworker’ to a group of people, taking lead responsibility for providing general, practical, and emotional support to promote their independence and wellbeing.
- Take a hands-on approach to help the person declutter and move belongings around their home which will involve some physical activity and practical support.
- Ensure that a comprehensive and up to date Person, Home, and Information Safety Profiles (PHISP) is completed and updated regularly on our internal systems, detailing all required and relevant information with regards to the person being supported, including all up to date safety management plans.
- Develop and maintain good local relationships with key partner agencies such as housing providers and where appropriate coordinate and chair professionals’ meetings to coordinate good outcomes for the people we support.
- Ensure all relevant outcomes, case studies and data collection requirements are completed to achieve satisfactory data monitoring and reporting.
- Ensure that on-going work, contact, personal information, important incidents, and developments are recorded accurately and as per our internal policy and procedures.
- Ensure any safeguarding concerns or disclosures are reported and recorded as per our local procedures.
- Adhere to H&S policies and procedures and ensure safety management plans are being followed and updated.
- Support the Service Coordinator in the ongoing development of the service.
- Participate in a lone working procedure to minimise risk when lone working.
- Raise awareness of services with relevant external agencies and in the community.
- Promote effective people involvement in activities wherever possible and appropriate.

- Car driver with access to own vehicle
