Are you passionate about making a difference? Hartlepool has a fully integrated substance misuse service, which includes our clinical service provided by an external partner and our ‘in-house’ care co-ordination service including a group work team.

If you are successful you will support the leadership team in both developing and delivering a service that we hope can improve the lives of people in Hartlepool.

As an Assertive Outreach Worker you will support individuals’ who have a substance misuse need and additional vulnerabilities within the wider community. The post holder will work to engage and encourage individuals to access services by breaking down barriers, tackling stigma and facilitating effective packages of care in conjunction with key organisations.

You will be part of a hardworking, enthusiastic and supportive team. We are committed to providing high quality services and offer an excellent working environment valuing supervision, training and opportunities for professional development.

Applicants should have experience of working with vulnerable individuals with multiple needs and possess a Diploma/BTEC/NVQ2/NVQ3 or equivalent qualification in substance misuse, psychology, counselling, social work or health and social care.
