You will be part of a multi-disciplinary team of Physiotherapists, Occupational Therapists, and Rehabilitation Support workers and work with the wider Community-based multi-disciplinary team, including Doctors, Nurses, Advanced Clinical Practitioners, HCAs, Mental Health Nurses Community Hospitals etc.

You will be supporting patients with rehabilitation and discharge planning. You will assist the Physiotherapist and Occupational Therapist with rehabilitation plans and completing assessments within your scope of practice.

This role cover rotation between WEB-Community Rehabilitation Team and Exmouth - in-patient rehab Therapy.
