As an Activity Coordinator you will work as part of the multidisciplinary team and support patients to identify and carry out leisure, social & vocational activities whilst on the Units and maintain associated patient records.

You will be able to organise, lead and facilitate a range of individual and group based activities and encourage patients to participate in an activity programme to support their rehabilitation.

The role will include you leading the day to day management of volunteers by acting as a contact point and to share and direct daily work plans. You will contribute to the development of our services to support patient experience within intermediate care.
- To plan, organise and co-ordinate the activity programmes for the unit.
- To provide a range of individual and group activities to motivate patients to participate.
- To support patients to identify their preferred leisure/ recreational activities using information from clients, families and MDT members as appropriate.
- To encourage and support patients to take part in activities suited to their particular needs and interests.
- To participate as lead or co-facilitator in group activities. With guidance from line manager, co-ordinate volunteers involved in supporting the ward activity programme
- To support and encourage patients to access local community, social and recreational facilities on discharge
- To provide information on available resources within their local community and from associated agencies.
- To organize activity and recreational events within the unit, e.g. group and individual activities.
- To demonstrate physical ability to carry out therapeutic interventions including manual handling therapy techniques and therapeutic handling.
