An opportunity has arisen within our enthusiastic and dedicated Cancer Services team for a permanent MDT Coordinator/Patient Tracker.

The MDT Coordinator/Patient Tracker is responsible for monitoring and tracking referrals with suspected cancer to ensure that patients’ appointments and treatment are booked to national and local cancer pathway targets.

The role is also responsible for facilitating and coordinating the functions of cancer site MDT meetings and ensuring that the meetings can run effectively and are well-structured. They will be required to live type the MDT minutes ensuring accuracy when doing so.

The role is responsible for facilitating and coordinating cancer site MDT meetings on a weekly basis. They will be working as part of a team of MDT Coordinators and trackers, who are committed to providing a first class service for patients in a fast-paced environment. They will also be a core and valued administrative member of the multi-disciplinary team and therefore must be able to communicate effectively and professionally with colleagues from a range of staff groups and hospitals.

To work primarily independently and on own initiative with mínimal supervision to carry out the duties of the post

Track patient test results to determine what further action is necessary, liaising with the clinician and/or ensuring that relevant information is available for discussion at MDT.

To be responsible for ensuring that the patient does not experience unnecessary delays on the cancer pathway.

Monitor the patients current status and communicate as required with the appropriate consultant to ensure that diagnostic/staging tests and treatment are arranged in a timely manner, and to chase results as required.

Supporting the PTL meetings

To communicate with colleagues in and outside of the Trust to ensure that patients are appropriately managed on their suspected cancer pathways

Utilise knowledge to determine which patient’s histology, reports and images will be needed for discussion

Provide administrative and technical support to facilitate MDT meetings; including but not limited to preparing, organising and circulating agendas

Live typing minutes from the MDT meetings ensuring accuracy and agreement from MDT lead before circulating as appropriate

Take responsibility following the MDT’s for ensuring that any outcomes identified are actioned in a timely manner, liaising with staff in various departments and other organisations

Work collaboratively with medical secretaries, clinic coordinators, nurse specialists and other staff within and outside of the Trust to ensure appropriate management of patient information and coordination of pathways

Assist in the review of office protocols and working practices where necessary.

Provide support with training for new staff where required, demonstrating duties to new starters.

Contribute to the cancer databases for patients (Somerset), updating records and contributing to the collection of data for audit and research purposes.

Be familiar with national Cancer Waiting Times standards and undertake training and e-learning as required

Be familiar with tumour site specific pathway information, including databases/spreadsheets or National Databases/Registries and enter data in a timely manner from the completed data collection proformas.

Implementation of the Operational Policy for relevant MDTs and Cancer Services. The post holder will also be expected to participate in meetings for the ongoing development of Cancer Services policies.

Attend and contribute to the tumour site AGMs.

To compile and provide detailed analysis of patient’s cancer pathways that have breached their target dates.
