This post is an exciting opportunity to join a multi-disciplinary Homeless & Rough Sleepers Service based in Blackpool to provide mental health support and interventions to a multi-disadvantaged client group.

The post-holder will work with adult service users who mainly have a mental health history, to provide longer-term intervention using a trauma-informed approach to care by identifying and implementing the most appropriate pathway and treatment plan for the client. The post-holder will work within a multi-disciplinary team to provide a seamless, consistent package of support that enables access to mental health, lived experience workers, physical health services, social care services, housing, substance misuse and all relevant partners.

The post-holder will provide line-management to junior members of staff, promoting mental health services to other providers and pro-actively working towards our Trusts values and objectives.

The post-holder will have a sound working knowledge of the Mental Health Act 1983, The Care Act 2014, relevant documentation and knowledge of Primary and Secondary Care Mental Health Services and Social Care.

The post holder will be highly skilled in communication, understanding, listening and engagement with mental health clients. The post holder will conduct high quality mental health assessments, care plans and conduct risk assessments, liaising with clients and colleagues working with the homeless / rough sleeping client group to ensure a multi-disciplinary team way of working is achieved.

The post holder will engage with the clients using a trauma informed approach to care and offer therapeutic intervention as appropriate.

The post holder will be responsible for maintaining high standards of documentation and data as required by the team and Trust, sharing information, meeting data protection laws and Caldecott guidelines.

The team is part of a multi-disciplinary, co-located service based in Blackpool and offers easy access to mental health intervention with an objective of improving the mental health and care outcomes for clients who have multiple disadvantages within the local community.

The post holder will be a specialist key member of the Mental Health Rough Sleepers Team, working alongside the Primary Intermediate Mental Health Team.

The post holder is to be effective in mental health assessment, formulating and providing intervention to people who are referred with moderate and complex mental health difficulties.

Liaison with various multi agency professionals from both statutory and voluntary organisations. Sign-posting referrals as necessary to ensure that clients access the most suitable care in a timely manner.

Working within a Trauma Informed model to support the complex and varied presentations of this patient cohort.

Act as a link person between the Rough Sleepers Team, The Primary Intermediate Mental Health team, GP’s, and Secondary Mental Health Services. This will mean facilitating the process of transferring urgent, or crisis work to alternative mental health services, jointly managing risk where appropriate.

Attendance at meetings in a professional, competent manner.

Contribute fully to team planning initiatives.

Submit statistical data according to team Key Performance Indicators.

Maintain contemporaneous and accurate written and/or electronic records.
