Band 7 Discharge Team Lead

Integrated Discharge at Imperial College Healthcare NHS Trust has as an exciting opportunity within the Charing Cross Integrated Discharge Team. We are looking for a passionate and dedicated individual who is expert in the area of complex discharge.
Within this role, you will work alongside the existing discharge team lead, to manage the team day to day, manage your own complex case load, be an expert point of contact for complex discharge issues and oversee the running of the discharge lounge.
As well as being a critical and highly valued member of Team Charing Cross, you will also be a part of the wider trust’s Integrated Discharge Leadership Team where you will work towards making trust-wide improvements to hospital discharge and flow with lots of opportunity to contribute to or lead on Quality Improvement projects and support colleagues on other sites. Our aim is to improve hospital flow, prevent delays, improve patient experience of hospital discharge and work collaboratively with internal and external stakeholders.

Operating within the framework of the Hospital Discharge Services guidance (Aug 2020), the post holder will work in partnership with colleagues in the acute sector, social services, continuing care and other community health services to ensure that patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
The post holder will be primarily based within the Charing Cross Hospital Integrated Discharge Team but will also work with the trustwide Integrated Discharge Leadership Team to promote best practice and ensure smooth complex discharge operations across Imperial College Healthcare NHS Trust.
The post holder will work within and across professional and organisational boundaries, liaising with Consultants, ward managers, other professionals and statutory and voluntary agencies to deliver individualised and expert assessment and discharge guidance to a high standard, and leading communication with all stakeholders.
The Integrated Discharge Team Lead is responsible for the development and the daily operational management of the discharge service (Integrated Discharge Team and the Charing Cross Discharge Lounge), including the line management, supervision and support of allocated staff within the team.
The post holder is required to work on the weekend rota as part of their contracted hours.

Patient responsibilities
- To lead and manage the complex discharge team on a day-to-day basis, as part of the Integrated Discharge Leadership team, providing clinical leadership, support and guidance.
- To assume responsibility for a caseload of patients with complex needs in a designated area within the Trust. To work autonomously to ensure that as soon as patients are medically stable, they are transferred to the appropriate discharge to assess pathway.
- To work in partnership with patients, their relatives and carers to develop optimum plans for discharge, providing resource information to assist with informed decision-making.
- To communicate effectively with the patient /carers to gain valid informed consent, prior to any intervention. To work within a legal framework for those patients who lack the capacity to consent to treatment.
- To work collaboratively with multi-agency professionals to develop comprehensive support packages for patients and their families living with long term conditions, both acute and chronic, ensuring appropriate service provision in response to changing clinical and/or social situations, including transition stages - end of life.
- To manage risk within own patient caseload and that of the team.
- To be responsible for maintaining accurate and comprehensive patient records in line with the legal requirements, Trust guidelines and professional standards of practice.
