**Job overview**:

- An exciting opportunity has arisen for an enthusiastic and experienced Nurse, Occupational Therapist or Physiotherapist to join the South Kerrier HomeFirst team, as a Homefirst Assessor based in Helston Community Hospital. _
- This position is open to Nurses, Occupational Therapists and Physiotherapists who have experience of working in the community and wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team. _
- The Homefirst team is part of the integrated community team in South Kerrier comprising of registered and non-registered nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population. _
- You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient’s condition/progress, identify problems and needs of patient and set realistic goals. You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person’s services. _

**Main duties of the job**:

- To provide a high standard Occupational, Physio therapy and Nursing to the service, performing advanced assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment plan.
- To operate as an advanced practitioner being first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.
- To utilise advanced skills in community assessment and management of the acutely sick patient.
- To provide leadership for support staff and junior staff through supervision, training and appraisal.
- To participate in the planning and development of occupational services within the designated team.
- Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.
