The postholder will ensure that residents are at the heart of the strength based conversations and planning process, finding creative solutions that enables adults to regain or maintain independence, access outcome focused support, and experience a seamless journey through the care system.

**Pay Rate**: £21.58 per hour (assignment rate)

**Contract**:Locum, 3-Months
