North Herts Integrated Community Team are seeking motivated and enthusiastic Band 4 Nursing Associate to join our friendly, supportive and Gold Accredited Team. You will be working alongside our health care assistants, qualified nurses and therapy staff assisting with the day to day care and management of our patients to ensure they receive safe, high quality nursing care.
As a health care assistant, you will work alongside our diverse team comprised of nurses, paramedics, physiotherapists, occupational therapists and technical instructors. We work to provide evidence based nursing care to patients in their own homes. Our service operates 8am - 10pm, 365 days a year. We are proud to offer flexible working patterns to support our staffing in maintaining a health work life balance; we are happy to consider full or part time applicants.

To utilise your extended skill set as a Nursing Associate to provide holistic, evidence based care to a varying caseload of patients within their own home.
To support your colleagues in the delivery of care and work with the wider team to promote the trust values of Caring, Agile and Innovative.
To work within a team that provides an assessment and care service to patients with nursing/health needs.
To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan. This will include aspects of wound care, catheter/bowel care, medication administration and more. The aim is to rehabilitate and maximise the patient’s potential and where appropriate to support their carer.
To support and enable patients in a range of activities to promote independent living and monitor their well-being.
To undertake administration and clerical tasks that are associated with the care process including record keeping and use of computer.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
