**About the Role**:
Responsibilities will include:

- To improve the lives of people with mental ill health by minimising the impact of their condition through the timely delivery of an excellent, compassionate service to promote the recovery and well-being of patients and their carers / families.
- To work collaboratively with general practitioners and other practice staff within the locality to address the mental health needs of their adult practice population.- To be professionally accountable and responsible for patient care, undertaking initial assessment and signposting to appropriate services or providing time-limited psycho-social/therapeutic interventions directly.- To carry own specialist caseload of complex cases and act as lead professional as appropriate, providing a pathway management function for other more complex patients within primary care.
- To promote social inclusion, community access and participation through the provision of patient care.
- To promote and maintain safety, privacy and dignity of all patients in the delivery of patient-centred care, recognising and respecting differences including spiritual and cultural beliefs.
- To act, wherever applicable, in accordance with the Mental Health Act, Mental Capacity Act and associated policies and procedures. To be responsible for recognising the potential for or signs of patient harm, abuse or neglect, including poor clinical practice, and to report all such concerns and take all reasonable steps to protect the patient. To be responsible for identifying and reporting concerns regarding the public protection and safeguarding of children and adults who may be at risk.
- To be responsible for ensuring the highest professional standards and attitudes towards the care of patients are maintained and that care is delivered in accordance with evidence based practice at all times. This will include providing oversight and supervision of practice-based mental health practitioners.
- To undertake a comprehensive initial assessment of referred or self-referred patients, including those with complex needs, and to use clinical judgement and analysis to signpost to appropriate services, or to retain on their caseload for time-limited therapy / interventions (average length of time on caseload expected to be 6 weeks).
- To support patients and their families/carers in the community, providing advice and liaising with other support services to co-ordinate individual support packages. This will include supporting practices where individuals have been admitted to hospital (eg through improved liaison between the practice and the hospital).
- To provide specialist clinical advice and consultation on complex issues to members of the practice team and staff from other disciplines and agencies, including advice on the clinical management of patients comprising prescribing advice as appropriate (within their sphere of competence and parameters of the NMP framework).

The following qualifications, skills and experience are desirable:

- Certificate in teaching & assessing in clinical practice as appropriate to own profession.
- Post-graduate qualification in a relevant area.
- Leadership or management qualification.
- Experience of working collaboratively with service users and their families/carers.
- Experience of working in primary care.
- Demonstrable leadership or management experience in a senior clinical role.
- Experience of providing clinical supervision.
- Mentoring students on practice placement experience.
