An exciting opportunity has arisen for a highly motivated individual to join Norfolk Community Health and Care as a Band 5 Keyworker Co-Ordinator, to support and work with Disabled children who have complex or high levels of need and who have a range of professionals involved. _
- As the key worker coordinator, you will be working in partnership with parent/carers and professionals across agencies and services, to ensure that children receive well coordinated services. _
- Identifying opportunities to develop the Key Worker Service and Pre-School Liaison Group, to ensure that local need is met. _
- This post is a substantive post for 37.5 hours per week, Monday
- Friday_
