**About the role**

This is a new and exciting role based on hospital wards at Lincoln County Hospital (and working in partnership with health and social care colleagues) the Discharge Buddies Coordinator will play a key role in supporting a person to achieve a timely and supported discharge from the hospital.

Enabling people who are medically fit for discharge to make informed choices about their health and wellbeing helps to relieve the pressure on the health and care system, allowing clinical staff to concentrate their efforts on those with a medical need.

This service will allow patients to be supported practically and emotionally throughout their stay with the added benefit of family liaison and carer support where appropriate. This will include:

- _Taking referrals from patients and ward staff for the Discharge Buddies scheme_
- _Completing home assessments to ensure the patient is accessing the support they need for a safe discharge home by arranging support with cleaning, shopping, deep cleans, prescription collection, laundry, travel to appointments._
- _Refer on to internal and external services where appropriate_
- _Marketing & promotion of services to increase take up_
- _Work towards achieving set KPI’s and Targets_

Each support package is tailored to each of our client’s needs and we provide as much or as little support as requested, ensuring our clients maintain their independence.
