**Hallam Medical have new opportunities for Community Mental Health Practitioners to support a Defense Community Mental Health team based in Brize Norton, Oxfordshire.**

As an independent practitioner you will work autonomously to meet the needs of military personnel with mental health problems by delivering biopsychosocial assessments across the spectrum of complexity, collaborative case conceptualisation and treatment plan(s), flexible and individualised care packages.

**Duties will include**:
Ensuring the provision of individual assessment, care planning, coordinating, evaluation and monitoring of specific evidence based therapies/treatments for patients referred to the service.

The maintenance of accurate and contemporaneous hard copy or electronic clinical records in keeping with NMC guidance on record keeping.

The provision of visiting community mental health liaison clinics at medical centres or other medical facilities within the defined catchment area.

This role is available on a part-time and full time basis with shifts ranging from 08:30- 16:30 (Monday to Friday only).

**Skills required**:

- RMN / RN (Mental Health)
- Post registration clinical training relevant to Mental Health
- Experience of working in a multi-disciplinary team
- Experience of mental health assessment, treatment, and evaluation
