Healthcare Assistant

To work as part of a multidisciplinary team to provide health care, and support under the direction and supervision of registered staff to patients living in the community. This includes assisting in the delivery of interventions, treating and discharging patients and reporting all changes in patient status to senior staff. The role also involves providing administrative support to the community team.
A job plan will be created by your line manager in line with the service Blueprint and discussed with you. It will be reviewed annually at your Performance and Development Review and updated as required. The main tasks you will spend your time doing are:
- Clinical Direct, for instance home visits, clinic appointments.
- Clinical Indirect, for instance patient documentation, travel to and from their home visit, and liaison with other professionals.
- Supporting Professional Activity, for instance imparting clinical knowledge, mentoring, giving and receiving training, and staff and team development
a)To undertake non-complex clinical procedures under the guidance of senior team members e.g.: venepuncture, patient specific sub-cutaneous injections ,sample collection, blood pressure monitoring, medications, wound care, and other tasks according to training and competencies.
b) To recognise their responsibility and accountability in accepting the delegated task.
c) Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team.
d) Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided.
e) To keep accurate, timely records according to Trust policy.
f) To escalate and report patient progress/ status on a shift by shift basis to senior team members.
g) To demonstrate flexibility and adaptability working across community teams according to service needs.
h) To commit to undertaking training and education to progress clinical skills as indicated by service development needs.
i) To respond effectively and actively to complaints and escalate appropriately following departmental policies.
j) To assist registered staff in care and treatment and participate in the review process.
k) To promote equality for all, including the maintenance of confidentiality, supporting individual customs, values and spiritual beliefs and ensuring that anti discriminatory practices are demonstrated.
