Here in Teesside, Durham and Sunderland, Home Group have a large number of support services, which help our many customers who all have individual and varying needs. Due to the expansion of our team we have a fantastic opportunity for an enthusiastic Support Coordinator like you, to join our Central Resource Team (CRT). Our team step in and help run our services when colleagues are on leave, absent through sickness or in emergency situations.

Instead of just working in one service, you’ll work with the varied teams across our region. You’ll have the chance to support our customers who have a wide range of needs across several specialised services. One day you might be supporting a customer with housing needs in the community, the next you might be working closely with our clinicians to get the best out of a customer with complex mental health needs.

As a Support Coordinator, you’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for customers. Each plan will look to meet the customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.

You’ll also have responsibility for coordinating our support workers and working collaboratively with external agencies, to assess and engage support for our customers.

**About you**

As a Support Coordinator, you’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

Ideally you will have some experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively. Don’t worry too much about this as training will be provided.

We are looking for adaptability and resilience and it would be amazing if you have worked in this type of environment before, however it’s your “can do” positive approach and enthusiasm that are critical to make this role a success. We can nurture your talent, just let us know in your cover letter why you have a passion for supporting people

To make sure that we keep our records straight, and customers safe, it's necessary that you have good IT skills (or at the very least be enthusiastic to learn)

Finally, as you’ll travel around the area to work in our services, it is essential that you drive and have access to a vehicle (we pay your expenses to cover this use).
- When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

**About our teams**

You’ll join our outgoing team, full of diverse characters, who are all friendly, helpful and supportive (great news for newbies). The team is led by Yvonne, who’s worked for Home Group since 2014. Yvonne started as a Support Worker and took full advantage of all the training and experience we offer here at Home Group. She is supported (and couldn’t do without) our Client Services Manager, Shannon, who started as a Support Worker within CRT.

**Working hours**

We have a variety of roles both part time and full time, rota based and Monday to Friday 9:00 - 17:00.
