NHS Devon are seeking to recruit 4 Specialist CHC Assessors across Devon. You can be based in either Exeter or Plymouth.

The post holder will be part of the Nursing and Quality Directorate for NHS Devon Integrated Care Board (ICB) their aim will be to improve the quality of care and value for money of services commissioned for individuals who are the responsibility of NHS Devon ICB.

You will be either a registered Nursing Professional or equivalent Allied Health professional/registered Social worker.
- To undertake NHS Continuing Healthcare (CHC) assessments of individuals who have been identified as potentially eligible for NHS funding contribution to their care. These assessments are to be undertaken in a timely manner and in variety of settings, including the individual’s own home, care home setting, virtual and other settings as required. You will also be required to travel to other locations across the county of Devon and on occasion outside of Devon to meet the needs of the service
