We are now recruiting nurses, pharmacists, physiotherapists and occupational therapists with an interest and experience in cardiology at various bands (from bands 5-7). We are looking for clinical staff who are passionate about the care they give to patients and who want to make a real difference in these areas. You will work in and be supported by, a multi-disciplinary care team including nurses, doctors, therapists, psychologists and each long term conditions will be supported with a Professional Lead to support with complex cases and to continuously improve the quality of service that is consistent across the patient’s journey.

This vacancy is for a Community Clinician with an Interest in Long Term Conditions.
1. Provide care to Long Term Conditions (LTC) patients as delegated by the Senior Clinician. Evaluate care plans and assess on-going needs within own level of competency.
2. Observe, record and report all changes to other members of the team within twenty-four hours
3. Awareness of current evidence based approaches to patient care and demonstrates this in practice.
4. Work in partnership with the patient to achieve optimum levels of self-care as agreed on the care plan.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
