Family Adolescent Team

**Job Description**:
The family adolescent support team is a new multi-disciplinary team working with young people 11+ where the main area of need is related to extra-familial harm. We also work with young people at the edge of care where there are concerns relating to mental health and substance misuse, young people 16 and 17 presenting as homeless, and supporting young people’s return home following a period of hospital admittance (tier 4)
The post will offer you the opportunity to:
- Work creatively and collaboratively within a multi-agency team and with partner agencies- Work intensively with young people and their parents/carers within a contextual framework.- Develop skill base and knowledge in relation to mental health, substance misuse, contextual safeguarding, and extra-familial harm.- Build strong and stabilising relationships with young people, and their parents/carers.- Work within a child in need framework, ensuring statutory timeframes are met and plans are outcome focused.- Offer support and guidance to Family Support Workers and oversee interventions.- Respond to risk in a timely manner and ensure appropriate safeguarding actions are taken.- Provide a seamless support service to young people up to the age of 25 years, transitioning into adulthood from Children’s Services into Adult’s Services and continuing to experience exploitation and its effects.- Embed arrangements for transitioning young people in partnership with a range of adult agencies and others that are comprehensive and inclusive in their approach to tackling exploitation and its consequences.**The Role**
Our team is locality based; this vacancy is for 1 Transitions Social Worker who will work across the county but have an office base in Rugby.
Across all areas our Social Workers will be:
- Passionate about achieving better outcomes for vulnerable children and their families, and this will be demonstrated through restorative practice- A role model for good social work practice- Overseeing and supporting Child in Need work undertaken by Family Support Workers- Review child in need plans and chair child in need meetings- A good knowledge base of the challenges associated with working with children and their families is essential- Applicants for the role will need a professional Social Work qualification and registration with Social Work England
