Key Responsibilities:

- Deliver exceptional support to individuals with learning disabilities, tailoring your approach to their support plan, cultural preferences, and aspirations.
- Guide and assist individuals in managing their homes, from maintaining tenancies to offering guidance during transitions.
- Provide personal care when needed, including toileting and bathing, ensuring that everyone’s dignity and comfort are upheld.
- Empower individuals to exercise their choices and make life decisions in alignment with the Mental Capacity Act requirements.
- Foster engagement in daily activities, from home tasks to social, leisure, and work pursuits, promoting personal autonomy and well-being.
- Support health needs, such as medication management and doctor appointments, promoting a holistic approach to care.
- Maintain accurate and up-to-date records, ensuring effective communication and continuity of care.
- Assist with financial matters following Hft guidelines, promoting financial independence and responsibility.
- Champion health, safety, and well-being, creating a secure and nurturing environment for those we support.
- Collaborate positively with colleagues, families, external organizations, and professionals to ensure comprehensive care.
- Embrace personalized technology opportunities to enhance individuals' independence, in line with current trends.
- Uphold confidentiality in all interactions, respecting the privacy of the individuals we support.
