**Band 6 Physiotherapist
**Knowsley community therapy team is seeking a committed Band 6 Physiotherapist to join the service, supporting the patients through shared care within their localities. We are seeking a highly skilled, enthusiastic, and proactive therapist to join ourteam. The service responds to the rehabilitation needs of adults 18+, who have multiple complex co-morbidities or support with discharge from the hospital setting. We are looking for an individual who can demonstrate experience within the Physiotherapy rolewho can work autonomously as well as part of a team. You will require sound decision-making skills and effective clinical reasoning to provide input to those with acute conditions, long term conditions, palliative patients, musculoskeletal problems (unableto access a clinic), orthopaedic and other neurological disorders.
Requires travel between sites. Can support a mix of remote/site working, will need to visit patients. Enhanced DBS.

Willis House, St Helens and Knowsley CLDT

**Band 5 or 6 Physiotherapist
**- The role is a community based Physiotherapy job within the St Helens and Knowsley Adult learning disability team. To fulfil this role you must the means and ability to independently travel to appointments across both St Helens and Knowsley.
- The LD Physio team currently accept referrals for adults who would be unable to engage in mainstream services. This may require the physio to make reasonable adjustments and use creativity to engage service users. Experience within the learning disabilityfield would be preferrable.
- To be able to independently and confidently asses and treat a varied range of physical complaints, with support from the band 6 physios This could include, for example purposes; new fall concerns, mobility decline, MSK issue or postural concerns on top oflongstanding deficits. A basic understanding of common neurological disorders would be preferrable.
- To implement an effective management programme for the current health issue. The Physio would be responsible to prescribe, monitor and progress the management programme. Use of outcome measures are essential to monitor effectiveness.
- Must have a good understanding into the MCA (Mental Capacity Act) and safeguarding processes.
- Requires excellent adaptable communication skills.
Part time remote work can be supported (cannot consider full time remote)

**Domiciliary physiotherapy provides short term Physiotherapy intervention to patients who are housebound and unable to attend an outpatient appointment. The caseload is varied, and will include frailty/falls, orthopaedics, MSK, neuro and more. The therapistwill need to be able to travel between patient’s homes and manage their own caseload with support from the wider team as needed.
Domiciliary role - will need to travel to patients home. Team work agilely - can work from home once visits completed. Enhanced DBS.
