An exciting opportunity has arisen for a motivated and dynamic dietitian, to join the community team at Hertfordshire Community NHS Trust (HCT) in East and North Hertfordshire, delivering a service as part of the Intermediate Care Team and Diabetes Team.

The post requires a full valid driving licence and access to a car to use regularly for business purpose is essential (unless you have a disability as defined by the Equality Act 2010).

This post would suit a soon to qualify or newly qualified dietitian, or one with experience.

The role includes providing a dietetic service to adults as part of the multidisciplinary team (MDT) diabetes clinics at East and North Herts NHS Trust. This is an existing post which includes working with adults who have Type 1 diabetes, gestational diabetes, group education sessions, and being part of MDT clinics at QEII, Lister and Hertford County Hospitals. The role also supports colleagues in the provision of a dietetic service in the delivery of general dietetic clinics using evidence-based practice and conforming to best practice guidelines.

This is a permanent part time post, 30 hours a week, with in-person clinics Monday and Thursday's, and Friday mornings.

**Job Description**:
To provide a dietetic service to adults as part of the multidisciplinary team (MDT) diabetes clinics at East and North Herts NHS Trust.

This work includes working with adults who have Type 1 diabetes, gestational diabetes, group education sessions including carbohydrate counting, and being part of MDT clinics (both face to face and virtual). Clinic sites are QEII, Lister and Hertford County Hospitals.

To support colleagues in the provision of a dietetic service in the delivery of general dietetic clinics using evidence-based practice and conforming to best practice guidelines.

To deliver group education sessions, virtually and potentially in person.

To be part of a dietetic rota for triaging referrals.

To provide education and training to dietitians, dietetic students, and other healthcare professionals (HCPs).

Main Duties

To provide a specialist dietetic service within the adult diabetes multidisciplinary team clinics in East and North Herts NHS Trust.

To provide expert dietetic advice as part of the diabetes multidisciplinary team and the general dietetic service, to ensure a high standard of clinical care.

To undertake comprehensive nutritional assessments of patients and make clinical judgements on a range of highly complex dietetic problems that requires investigation, analysis and assessment.

To formulate individualised management and treatment plans autonomously, using clinical reasoning, and utilising a wide range of treatment skills and options to formulate a programme of care.

To utilise advanced communication and teaching skills.

To work within a legal framework, following organisational and departmental policies and procedures, consider ways to improve patient experience and to promote the health of patients, carers and members of the public.

To provide leave cover when other members of the dietetic team are absent, according to clinical competencies and on an agreed cover.
