The Midlothian Intermediate Care team supports patients who have not been fully assessed by the inpatient AHPs and who require urgent input at home to complete the assessment process and facilitate discharge by providing rehab at home. The team also completes crisis prevention & rehab to prevent hospital admission.

Working shift patterns to include evening and weekend working will be required. The ability to travel throughout the area in a timely manner is essential to support the clinical and rehabilitation needs of our patient group.

The clinical support workers play a key role within the team by supporting patient’s to complete activities of daily living such as personal care, meal preparation, medication management and enabling patient’s to return to independence. The clinical support workers will work closely with the therapists who will complete assessments for patients in their own homes and provide rehab/care plans for the clinical support worker to follow.
