Do you have experience as a GP Medical Receptionist and you're looking for an opportunity to use and develop your skills?

The practice Care Coordinator (PCCs) will be responsible for providing the link between the duty doctor, the reception team and the patient on a rotational basis. They will use their knowledge to provide care navigation to patients and experienced support to the rest of the reception team.

They will be required to ensure that our patients' changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. This will also include ensuring that an individual’s care is provide by the practice in as efficient way as possible.

The PCC’s role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the practice including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, dieticians, occupational therapists and pharmacy technicians.
- Help people to manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care, using tools to understand people’s level of knowledge and confidence in skills to manage their own health
- Support people to take up training and employment, and to accessappropriate benefits where eligible
- Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives
- Provide cover for the reception team
- Assist in reception team training and “buddying”
- Develop and maintain knowledge in the Practice administrative functions (e.g. prescriptions and referrals)
- Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals
- Undertake all mandatory training and induction programmes
- Contribute to and embrace the spectrum of clinical governance
- Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
- Contribute to public health campaigns (e.g., COVID-19 or flu clinics) through advice or direct care

**Job Types**: Full-time, Part-time
Part-time hours: 25-37 per week

**Salary**: £11.50-£12.50 per hour
