Job Title: Band 6 Community Nurse Location: Cumbria, United Kingdom Salary: £35 per hour

Are you an experienced and passionate Community Nurse looking for a rewarding opportunity? **Responsibilities**:

- Support patients and their families in managing their healthcare needs, offering guidance and education.
- Collaborate with multidisciplinary teams to coordinate and deliver holistic care.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Maintain accurate and up-to-date medical records and documentation.
- Promote health and wellness through health promotion and prevention strategies.
- Participate in clinical audits and quality improvement initiatives.

**Requirements**:

- Registered Nurse with a valid NMC (Nursing and Midwifery Council) PIN.
- Proven experience as a Community Nurse, preferably at Band 6 level.
- Excellent clinical skills and knowledge of community nursing practices.
- Strong communication and interpersonal skills to engage with patients, families, and colleagues effectively.
- Ability to work autonomously, managing a caseload and prioritizing care.
- Sound understanding of relevant legislation, policies, and procedures.
- Commitment to professional development and continuous learning.
