Stroke Flow Coodinator

We are seeking an energetic, dynamic, and enthusiastic individual with strong organisational skills for the role of Stroke Flow Coordinator for Salford Royal.

The purpose of the Stroke Flow Coordinator role is to ensure, with nursing and medical colleagues, that patients, ward admissions and investigations are effectively coordinated across site.

You will be responsible for liaising with all members of the MDT both here at Salford and across Greater Manchester. You will ensure effective communication about patients fitness for transfer status, problem solving, capacity management and appropriate early escalation to managers.

You will be responsible for the safe and timely discharge of patients from an allocated clinical area, working within the clinical team to establish needs, monitor pathways, plan, and help deliver a safe and timely discharge. This will be achieved through communicating with patients, relatives, and assisting with completion of all relevant referrals and clinical documentation within target timeframes.

You will co-ordinate daily flow onto and from the stroke units at Salford and achieve early movement into vacated beds to support flow from the emergency department. You will also support the operational site team by providing regular updates, potential discharges, and bed availability.

You will maintain information on fitness status, delays in transfer and keep the stroke database up to date. You will send out regular site report throughout the day. You will work closely within a multi-disciplinary team.
