i) Following plans of care and reporting to a more senior member of staff to undertake a range of duties including the delivery of recovery focused interventions. Working with service users who are homeless, doing outreach work. Direct supervision or guidance from the team manager. ii) To be aware of and assist in meeting the psychological, social, physical, recreational and spiritual needs of all service users.
To recognise and value service users as individuals, recognising the importance of maintaining the service users dignity at all times. To ensure that the care provided respects equality and diversity that is supportive of service users and their carers. iii) To maintain timely and accurate patient written / electronic records under supervision and in line with Trust and service line policy and procedure iv) To respond to service user needs without delay, identifying and working to tackle obstacles as they arise, even if they are of another agencies making. v) To use discretion to tailor support so that it is proportionate to the service users needs and individualized to their circumstances.
vi) To act as a key contact point for service users, drawing in support as required. vii) To support the assessment and management of patient / service user care needs through initial data collection; feedback; discussion and reports and in care reviews with the team. viii) To undertake and assist in the physical health care assessment and or on-going physical health monitoring / review of service users, recording any changes in the service users physical health, reporting these to a more senior member of staff, acting appropriately on the advice given.
