If you are a Self -motivated, dynamic individual with a passion for community care, this could be the position for you.

You will be working alongside a multi-skilled team of community practitioners led by Specialist Practitioners in District Nursing. Your role will be to support the team with service delivery supporting with clinical interventions and providing care to vulnerable individuals.

The District Nurse team promotes wellbeing and independence to all our patients in an understandable and approachable way. You must be able to display effective communication skills with patients, family, carers as well as the wider Multi professional workforce.

The post holder will work within a Multi Disciplinary Team in the Community and after period of induction will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care, both for a specific group of clients as the named care provider and in supporting the care of all client groups within the community.

You will act in accordance with locally agreed standards of care and ensure that clinical procedures are correctly carried out. As an experienced nurse and as appropriate, you will co-ordinate the case load in the absence of the team/senior lead ensuring that effective communication is maintained at all times, providing support for junior staff within the team. You will pay a key role in the supervision and assessment of pre and post basic learners and unregistered staff providing a teaching role and acting as named supervisor.

Main

**Responsibilities**:
Answer the telephone, take messages and deal with enquiries on behalf of the team. It may be necessary to take appropriate action e.g either pass on the information to a specific staff member, return the call or make a note in the message book.

Input data onto spreadsheets

Ordering equipment, requesting collections and maintaining stock levels

To ensure that equipment is fit for purpose and to report any defects to qualified staff.

To accompany a team member during domiciliary visit

To attend departmental staff meetings and mandatory training sessions.

Ensure safety of self, colleagues and patients.

To take part in the appraisal process as an attendee.

Comply with the requirements of the Data Protection Act, Access to Health Records Act and legislation in relation to confidential data

Adhere to Trust Policies and procedures such as the Infection Control Procedure

Communications

**The post holder will need to**:
Develop working relationships with the immediate team and across the wider District Nursing service.

Communicate with a wide range of people to ensure that planning of work and the allocation of appointments is communicated appropriately

Communicate confidential, sensitive information to patients as directed by team members

Use appropriate communication skills to enable you to deal with challenging people or situations as required

Ensure that all locally produced information documents are in the corporate style and that all written information complies with Trust policy

Responsibility for Patient Care

Ordering appropriate equipment for patients as identified by trained team members

To accompany another team member during domiciliary visits if required to

Assist patients/service users/carers/relatives during incidental contacts

Planning and organising

**The post holder will**:
Organise and plan their own day to day workload, in order to meet the demands of the service and job role

Support team members with planning of work and coordination of visits as requested
