'

**Purpose of role**
- To work as part of the care home team, to positively promote recovery and social inclusion, by focussing on the needs of Service Users.
- Contribute to the ongoing support of Service Users, promoting their rights, independence and individuality.

**Key responsibility**
- Assist in all areas of care in accordance with agreed care plans and procedures, to enable Service Users to meet their individual needs, including activities of daily living, household tasks and social and leisure activities.
- To administer prescribed medication as needed.
- Contribute to the development and evaluation of care plans, under the supervision of a Registered Nurse.
- Contribute to the prevention and management of behaviour that challenges, by employing positive behavioural support approaches and techniques if and when appropriate.
- Assist the Care Home Manager and Nurses with ordering of medication.
- Assist the Care Home Manager and Nurses with delegation of tasks, throughout any given shift to ensure best use of staffing resources.
- Form positive therapeutic relationships that support Service User’s own decision making and builds on their strengths.
- Conduct, record and report observations and ensure care plans are implemented and quality standards are achieved.
- Monitor the wellbeing of Service Users, which may include, though is not limited to, undertaking physical health checks, blood pressure, temperature, respirations etc...
- Encourage and support individuals to take part in activities suited to their particular needs and interests, ensuring facilitation of meaningful activities for Service Users.
- To facilitate/co facilitate groups and activities.

**DUTIES AND SPECIFIC RESPONSIBILITIES**

**Manage the safety and quality of the service**

1.1 Under the supervision of the Senior Team, promote the delivery of the safe service in accordance with legislative requirements and Company Policies and Procedures.

1.2 Report immediately to your line manager any illness of an infectious nature or accident incurred by a Service User, colleague, self or another.

1.3 Understand, and adhere to the implementation of, the Company’s Health & Safety Policy, and Emergency & Fire procedures.

1.4 Report to the Maintenance person, any faulty appliances, damaged furniture, equipment or any potential hazard and ensure any corrective action is taken.

1.5 Implement effective practices that support compliance with health & safety at work, infection prevention and control, food hygiene/environmental health regulations and all other regulations.

1.6 Maintain accurate, legible and relevant records of Service User support, under the supervision of the Registered Nurse and Managers.

1.7 Adhere at all times to the Policies and Procedures of the Company.

1.8 Report all complaints and incidents to the Nurse in charge without delay.

1.9 Be prepared to work flexibly to ensure the safe delivery of the service.

**2. Provide High Quality Service Provision**

2.1 Promote the rights of each Service User and keep their wishes at the centre of their care and support.

2.2 As part of the role of the Senior Recovery Worker, participate in pre-admission transition plans to support effective admissions.

2.3 Support Service Users to participate in a range of activities that are meaningful to them and meet their needs and choices.

2.4 Support Service Users, and where appropriate their representatives, to understand the service provision, and where you identify any areas of misunderstanding, report these to the Nurse leading the shift.

2.6 Keep all information about Service Users and their families secure and confidential.

2.7 Support the delivery of Service action aimed at promoting the quality of service provision.

**3. Leadership and Management**

The post-holder will complete the required social care training to enable them to fulfil their role effectively.

3.1 Support the induction and training of new team members, acting in the ‘buddy’ role as required.

3.2 Support the identification of your own ongoing staff training needs, ensuring you remain in date with all mandatory and other required training.

3.3 Participate in ongoing supervision to support you to effectively and safely carry out your role.

3.4 Participate in an appraisal and implement objectives agreed at the appraisal meeting.

3.5 Comply with Safeguarding Adults legislation, reporting any concerns without delay and participate in related investigations, as required.

**4. Promote the business**

4.1 Deal with any enquiries for a placement professionally and positively and inform the Nurse/ Care Home manager of the enquiry in a timely manner.

**5. General**

5.1 Contribute to the development of an effective and efficient working team.

5.2 Develop and maintain a network of professional contacts with all stakeholders.

5.3 Report immediately to your Line Manager any issues identified which could bring the Company into disrepute.
