We have an exciting opportunity for a Band 4 Diabetes Educator to join our friendly team within Harrow Community Diabetes Service. This is an exciting time to support people living with diabetes and to help facilitate the management of their condition. The post holder is expected to deliver and facilitate Diabetes Structured Education DESMOND for people with diabetes in Harrow and Brent Community. Full training and support will be given for the role.

DESMOND stands for Diabetes Education and Self-Management for Ongoing and Newly Diagnosed. The DESMOND Newly Diagnosed & Foundation modules (ND/F) are part of an established education programme designed to give people with Type 2 diabetes a good start in managing their condition.

In the DESMOND ND/F education courses two trained Educators work together to facilitate group education for people with diabetes. Previously, only registered healthcare professionals (e.g. diabetes nurses, dietitians) could take on this role. A successful research study, which trained non healthcare professionals with an interest in diabetes, has demonstrated that non healthcare professionals can be trained to deliver education courses with a healthcare professional partner, to the same standard and with the same positive results as those using a team of 2 healthcare professionals.

An Educator (non-HCP) in DESMOND must follow a prescribed training programme as set down by the DESMOND National Programme. An Educator (non-HCP) will deliver DESMOND diabetes group education together with a healthcare professional Educator partner, who has been accredited by the DESMOND National Programme.

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.

To follow the prescribed training programme for DESMOND Educators (non-HCP), which will include:

- Observing a local DESMOND education course. Attending 3 separate days of training organised by the DESMOND National Programme which may involve travel in order to attend
- Committing to a mentorship programme delivered locally by an assigned mentor (an experienced and accredited healthcare professional DESMOND Educator)
- Using the development workbook and other resources to support ongoing development as a DESMOND Educator
- To deliver the DESMOND Newly Diagnosed & Foundation modules together with an accredited healthcare professional DESMOND Educator
- To work as part of a team to effectively deliver the DESMOND ND/F modules according to the curriculum and using the appropriate resources
- To ensure patient confidentiality at all times
- To participate in personal development and reflection through giving and receiving feedback on performance as an Educator
- To be prepared to study in own time (e.g. using development workbook, curriculum and training materials and preparing for sessions)
- To carry out self -evaluation after delivery of every DESMOND education course
- To participate in the DESMOND Training and Quality Development Framework
- To Support the ordering, monitoring, maintenance of stationery, stock and resources
- Liaise with other relevant agencies /departments for booking interpreter
- To participate in on-the-job learning, this includes shadowing experienced staff, reading up on the service and CLCH policies, asking for guidance, and asking questions to clarify points of limited, knowledge, experience and understanding
- Undertake periodic performance and competency reviews.
- Responsible for own personal development
- In addition to mandatory training, staff may be requested to attend fire and first aid training if required by the service
- To undergo a Criminal Record Bureaux (CRB) check.
