**About the Role**:
Broxbourne Alliance have an exciting new opportunity to join our care coordinating team within our Primary Care Network, supporting 4 GP Practices across the Borough of Broxbourne in Hertfordshire.

About the role:

- Proactively identify & work with a cohort of patients to coordinate & navigate the support with their personalised care requirements.
- Ensure that patients have the correct information to help them make informed choices about their care.
- Liaise with stakeholders & other agencies to coordinate pathways of care for patients.
- Work with other PCN staff towards PCN directed priorities & targets helping to coordinate system wide care within Primary Care as necessary.
- Support our practices with Quality & Outcome Framework & other priorities & targets.
- To undertake support for patients who have been referred to the suspected cancer pathway from Primary Care & to help increase uptake of Cancer screening for all appropriate patients.

Skills/Experience.
- Delivering person centred care support in a non-judgemental way.
- Experience of working with vulnerable groups (paid or voluntary).
- Excellent interpersonal skills & the ability to interact well with different kinds of people.
- Ability to maintain a high level of confidentiality & respect for patients.
- Passion for helping people & providing personalised care.
- Excellent verbal & written communication skills including being able to handle multiple tasks at a time.
- Proficiency with computers & Microsoft office tools, such as Word, PowerPoint & excel.
- Strong leadership skills & the ability to motivate others
- Understanding of organisational policies and medical regulations.

Broxbourne Alliance is a busy Primary Care Network supporting four local practices; Abbey Road Surgery, Cuffley and Goffs Oak Medical Centre, The Maples Health Centre, Warden Lodge Medical Centre.
