Our fabulous #TeamTransitions are expanding and are looking for outgoing and committed individuals with a creative flair to join them as Support Practitioners on a permanent basis. **We have various contracts available, up to 39 hours per week.**

It would be great if you have experience with Autism and learning disabilities however this is not a deal breaker as we recruit on values and transferable skills; we're more interested in what you as a person will bring to our team We'll provide full training and give you all the practical skills you'll need to build an amazing career within the Health & Social Care sector. We'll even fund your SVQ/SCQF qualification too

Our fabulous team provides full 24 hour support to children and young adults between the ages of 10-26, this could be in their own home and to those at school to offering alternative to respite weekends away, regular weekly supports evenings and weekends. Each support is tailored to the needs of each individual and could include supporting young adults leaving school to access further education with 1:1 supports at college along with support to access the community.

We are looking to fill the following contracts;
- 1 x 16 hour contract who will support a mixture of young adults and school age children during the week and every second Saturday.
- 2 x 20 hour contracts supporting two young men (age 18 and 19) to attend college during the week and also access the community during the summer.
- 2 x 39 hour contracts supporting a young man moving in to his own home, can display behaviours that challenge and is supported on a 2:1 basis, shift pattern will include sleepovers and weekend working.
- 1x16 hour contract supporting a young man, who resides with family, to access the community attend groups which includes swimming. His support is 11:00-15:00 Monday-Friday and one weekend a month where sleepovers are required.

**For these roles we will need you to hold a full manual driving licence. **Access to a car would be great but this is not essential as many of the people we support have their own vehicles.

**All we’ll need you to bring is;**
- Full UK driving licence
- A caring and compassionate attitude
- The ability to work well in a team
- Good communication and organisational skills
- A positive attitude with a willingness to learn
- A creative drive to improve the lives of the individuals you support
- Confidence in the use of computers

**Our sleepover shifts do not form part of your contracted hours, so you will receive an additional payment of £105 per sleepover.**

If you’re successful in joining our team, you will be expected to join the Protecting Vulnerable Groups (PVG) Scheme and must be willing to work towards a SVQ in Social Services and Healthcare as required for SSSC registration. We will need you to register with the SSSC within 6 months of starting your employment with us.
