**Job overview**:
We are looking for a caring, motivated Health Care Assistant to join our professional and dynamic team. With the increasing number of people choosing to remain in their own homes for their end of life care, we are looking to expand and develop our existing service. In particular you will be delivering the highest standard of care and support to people who are approaching the end of their lives and their families.

A car driver is essential.

Full time hours.

This is a lone worker post.

**Main duties of the job**:
You should be able to demonstrate a real enthusiasm for palliative care, an ability to deliver care to patients in their own homes, possession of appropriate or transferable skills as well as a desire for ongoing professional development.

You will be expected to travel independently across the Central area to fulfill the requirements of the post. Mileage is paid at Agenda for Change rates or a base car is available. Travel time is included within your working hours.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Just over 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
