FIXED TERM CONTRACT - 18 MONTHS

Monday to Wednesday - 22.5 hours per week.

Physical health checks for people on the SMI (Serious Mental health Illness) register is a key target relating to the NHS Long Term Plan and one of the measures being driven as part of the Community Mental Health Transformation agenda. Previously the ICS has not hit this target, so it was identified as an area for support.

Aims:

- Support agenda to ensure all patients on the SMI register have a physical health check every 12 months.
- Support recovery agenda to ensure those missed during Covid-19 are caught up.
- Reduce health inequality experienced by patients with serious mental illness when compared with the general population.
- Potential to refer relevant patients for ongoing remote monitoring to further support population health management.

This role will be based at West Cumberland Hospital with a requirement to travel to Carlisle at times.
