An exciting opportunity has arisen within our Patient Flow team that is responsible for coordinating both early and complex discharges from our acute inpatient wards, minimising delays and maximising bed capacity.

We are looking for an individual with a good understanding of acute inpatient discharge who will be committed to driving the discharge process, identifying and anticipating any issues that may arise, working closely with the MDT and developing effective relationships with both internal and external agencies.

You will need to be enthusiastic, have good problem solving skills, be creative and innovative in your approach, work well under pressure and be able to maintain effective relationships where conflict may rise.

Encouragement will be provided to develop your skills, knowledge and competence through regular supervision, training and continuing professional development.

The role will involve traveling to non KMPT hospital sites so car business insurance is desirable.

Working with the team to support the discharge of patients, and particularly complex patients who may need support to be discharged to another health setting such as a locked rehab facility. Working closely with the MDT to ensure all patients have realistic estimated dates of discharge, and ensuring any barriers to discharge are quickly identified.
