An exciting development opportunity has arisen to join our multi-disciplinary team as a band 4 discharge coordinator.

The role of the discharge coordinator will be to actively and independently monitor the multi-disciplinary management and discharge plan for all patients to ensure efficient, timely discharges.

Our Community Hospital is in the beautiful Market Town of Stroud in a semi-rural position in the Cotswold’. It has the bonus of good access to the M5 motorway and is also serviced by excellent train links to London, Bristol and the West country.

You will be expected to work closely with all members of the multidisciplinary team, including physiotherapy, occupational therapy, adult social care, nursing and medical staff, mental liaison nurse and the patient flow team. Externally you will need to have working relationships with relatives, care agencies, district nurse teams, telecare, the Carers Hub and Age UK, to name but a few.

The post is full time 37.5 hours.

The job involves taking an overarching role in supporting patient flow through the system. A critical success factor in this role is reducing the length of stay for all patients. To enable a patient to achieve their optimum level of independence and rehabilitation potential.

You must therefore have excellent communication skills and be confident to work on many levels within the MDT as well as external community organisations. In return we can offer you day to day support and a real commitment to your personal development plan.

We are looking for a highly motivated individual, with experience of rehabilitation within a hospital setting and possibly community working, who would like to develop their career in a friendly and local environment.
