Louth, United Kingdom Lincolnshire Community Health Services NHS Trust Full time

Are you passionate about delivering a high quality patient centred care?

Are you motivated and dedicated to supporting patients with the transition from hospital to home?

Do you want an opportunity to work in a service that is transforming how care is delivered in our community?

If so, then this could be the job for you

We are looking to recruit a highly motivated Rehabilitation Assistant to join our friendly and supportive team based on Archer ward at Louth County Hospital Louth.

The therapy team provides care and rehabilitation for a variety of special areas including orthopaedics, neurology, frailty and falls, chronic disease management and other medical conditions. The service provides high quality care and works to an enabling philosophy to facilitate patients to achieve their maximal level of independence thereby facilitating discharge and reducing length of stay in hospital.

Lincolnshire Community Health Services NHS Trust is in the top 25 per cent of all community trusts for overall staff engagementin the 2021 NHS National Staff Survey.

LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Flexible working opportunities such as part-time working, compressed hours, job shares, and hybrid working models can help staff to balance their personal and work commitments. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.

Your role will include assisting patients to regain as much functional independence as possible through working on a patient rehabilitation goal approach. The post holder will be able to use skills to carry out simple assessments, follow care plans and prescribe routine equipment within the scope of the job and to a level of assessed competence. Part of this post involves working, without direct supervision at times but under the professional guidance of occupational therapists and physiotherapists.
