Home 1st are excited to advertise a new Mental Health post. This innovative post will support the mental health needs of patients in the Home 1st service which is the umbrella term for 4 physical health teams - UCR, Admission Avoidance, Community Therapy and Reablement services.

The service needs another motivated, dynamic practitioner to help continue this work providing an exciting new service to this well-established, friendly team.

The post covers all teams within Home 1stwho’s primary aim is to help people remain at home, providing short-term intervention.

You will work alongside UCR Nurses, Physiotherapists, Occupational Therapists, Therapy Assistants and a Clinical Psychologist.

The MHP will deliver learning sessions about a range of mental health diagnoses and management for all staff within Home 1st. Additionally, informal discussions with clinicians around patient’s presentation is key to providing an informed and well-rounded service.

You will be provided with ICT laptop, mobile phone, uniform and have mileage expenses paid for.

The Home 1stMental Health Practitioner will provide a high level of expertise and clinical care to prevent the unnecessary hospital admission of acutely unwell patients including nursing and residential care home environments.

The Home 1stMental Health Practitioner will work across Urgent Community Response (UCR), Admission Avoidance, Community Therapy and Reablement services to meet the needs of the patients referred to the Home 1st service:

- Accepting referrals from Home 1st clinicians where Mental Health expertise is required
- Providing advanced holistic assessment and expert clinical care including short term monitoring of patients during an acute period of illness
- Provide a specialist resource for people with an acute illness and support relatives and carers
- Provide a specialist mental health resource for people using the service as required and offer support to their relatives and carers.
- Provide mental health advice to community health colleagues in Home 1
- To support and ensure smooth transition of the individual between community and Mental Health services where required
- The post holder will be an experienced qualified mental health professional Nurse / Occupational Therapist) with relevant experience in the care and treatment of older adults.
- To support, advise, signpost or refer onto appropriate services when indicated.
- Knowledge of care of Older Adults with a diagnosed mental health disorder, both functional and organic (dementia)
