**Support to Older People and Carers**:Provide assistance to older individuals and their caregivers to enhance their independence.

**Information Handling and Advice**:Receive, gather, and provide information to individuals, offering advice as needed.

**Assessment and Support Planning**:Assess customer needs and determine eligibility for services.
Develop and implement support plans, regularly reviewing and adjusting as necessary.

**Record Keeping**:Maintain accurate records in accordance with departmental policy.

**Information Gathering**:Collect relevant information from customers, families, and other agencies regarding the needs of individuals requiring assistance.

**Information Provision**:Provide accurate information to individuals and direct them to other agencies and resources.

**Assessment and Reviews**:Conduct assessments and reviews under the supervision of the Senior Practitioner/Social Worker.

**Options Identification and Support Planning**:Identify options to meet recognized needs and collaborate on a support plan, considering eligibility criteria.

**Support Plan Implementation**:Ensure timely implementation of the support plan.

**Monitoring and Outcome Assessment**:Monitor the needs of clients and their caregivers to ensure positive outcomes.

**Professional Collaboration**:Liaise with other professionals (e.g., GP's, District Nurses) to determine appropriate actions.

**Caseload Management**:Manage a caseload effectively and in a timely manner under supervision.

**Meeting Attendance**:Attend reviews, case conferences, and professional meetings as required.

**Record Keeping**:Keep accurate and timely records in line with departmental policy.

**Interpersonal Skills**:Use communication, reasoning, and negotiation skills to establish and maintain positive relationships with staff, customers, and their carers.

**Confidentiality**:Ensure customer confidentiality is maintained at all times.

**Supervision and Review**:Participate in monthly supervision and annual Personal Development Reviews.

**Team Collaboration**:Assist in the general workload of the service and support colleagues under the guidance of the Senior Practitioner/Social Worker.

**Training and Development**:Attend mandatory training and participate in staff development courses as appropriate.

**Team Meetings and Policy Compliance**:Attend team meetings and ensure compliance with departmental policies and procedures.

**Additional Responsibilities**:Undertake any other duties of a similar level and responsibility, as directed by the Advanced Practitioner/Team Manager.

This role involves a combination of direct client interaction, assessment and planning, collaboration with other professionals, and adherence to policies and procedures in the field of Adult Services. It requires strong organizational and interpersonal skills, as well as a commitment to maintaining high standards of care and professionalism.

**Salary**: £23,213.76-£46,532.45 per year
