The ageing population and increasing rate of dementia diagnoses means that, an estimated 6000 people will be living with dementia. The Memory Link Worker is an exciting role, developed in response to the Welsh Government Dementia Action Plan, offering a unique opportunity to make a real impact on the well-being of people with dementia.

The job is to provide person-centred support to people diagnosed with dementia and their carers living in Cardiff and the Vale Localities to help improve well-being and day to day functioning, solve practical issues to ensure timely and co-ordinated care and support from dementia diagnosis to end of life.

The link worker will work with people with dementia and their carers, on an individual basis, acting as a long term advocate and single point of contact for them.

The link worker will also liaise with other hospital, community and third sector services as appropriate

The role will also require proactive engagement and promotion of this service to primary health care professionals within Cardiff & Vale and working in partnership with the person and their carers, the Locality and Specialist Memory Team to ensure that their needs are heard and addressed in a timely and appropriate manner.

The successful applicant will ensure that best practice is reflected and acted upon, whilst working proactively with the cluster GPs, the Locality and the Specialist Memory Teams and others to promote the service and stimulate referrals. It is a highly challenging role, but one that has huge rewards and has a significant impact on the patients that it provides help for.

Please note this post is for a Memory Link Worker, fixed term for 15 months to meet the needs of the service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
