**Role Summary**:
AFC Fylde Community Foundation is looking to recruit a project co-ordinator for their Fylde Loneliness Prevention Project that will support older people in the Fylde borough to reduce social isolation and loneliness.

**Guidance and Authority**:
The Loneliness Prevention Project Coordinator will report directly into the Assistant Foundation Director.

**Main Duties**:

- Co-ordinate the overall delivery of the Loneliness Prevention Project ensuring the project reaches and surpasses all targets, as set out in project plan.
- Receive and allocate all incoming referrals from project partners and provide consultations with individuals in community settings.
- Build trusting relationships and provide personalised support for individuals allowing them to take control of their wellbeing and improve their health outcomes.
- Produce a personalised support plan for individuals to improve their health and wellbeing, introduce or reconnect people to community groups and statutory services.
- Liaise and communicate regularly with local partners including carers, health and social care professionals, voluntary sector and stakeholders involved in the wellbeing of your caseload.
- Co-ordinate the full monitoring and evaluation of the Loneliness Prevention Project reporting against all Key Performance Indicator’s and compiling evidence on the progress of individuals.
- Ensure all activities are correctly monitored with the appropriate risk assessments and health and safety procedures put in place.
- Work with the Assistant Foundation Director to identity new funding opportunities and partnerships.
- Be a flexible member of the AFC Fylde Community Foundation team and when requested assist in the delivery of other projects and activities including evenings and weekends.

**Please note**:The role will require managing your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the project - e.g. when there is a mental health need requiring a qualified practitioner.

**Health and Safety**:

- Fully endorse, demonstrate, and carry out the Foundation’s health and safety policy.
- Comply with all group policies and statutory regulations relating to health and safety, safe working practices, hygiene, cleanliness, fire and COSHH. This will include your awareness of any specific hazards in your workplace and assist if required with the amending of risk assessments periodically.
- Have a full knowledge of procedures for evacuation with regards to fire safety.
- Identify and report maintenance requirements or hazards within the workplace and encourage any workforce to do the same to avoid injury.

- The Foundation is committed to safeguarding the welfare of children and young people and expects all staff and volunteers to endorse this commitment.
- The employee must act to protect all young people and vulnerable adults that are in their care or attending the group’s premises. The employee must report any misconduct or suspected misconduct to the Foundation Director.

**Person Specification**

**Qualifications & Experience**
- At least 2 years’ experience of working in voluntary sector, health, or social care in direct contact with people, families and carers in a paid or voluntary capacity
- Experience of working towards key performance indicators
- Emergency First Aid
- Hold a safeguarding qualification or evidence of recent professional training
- Experience of working with adults to achieve successful outcomes
- Proven track record of working with vulnerable adults and affecting positive change in behaviour
- Experience of working alongside key agencies across heath and social care and the voluntary sector
- Qualification in support working or counselling
- Experience of working with volunt
