The main duties include, taking part in care navigation telephone triage on a daily basis ensuring patients are navigated to the correct clinician or clinical service, provide support for the reception team and clinical team to ensure the correct processes are followed, action tasks assigned to the care navigation team and attend regular care navigation meetings. Experience desired but not essential as full training will be provided.

**Salary**: £10.25 per hour

Hours: 25 per week, hours will be 08:00 -1pm. Monday-Friday

The interview process will consist of a telephone interview to assess communication skills and a face-to-face meeting.

**Job Types**: Permanent, Part-time
Part-time hours: 25 per week

**Salary**: From £13,325.00 per year

Schedule:

- Day shift
- Monday to Friday
