The key purpose of the role of the post is to work within the guidance provided by the National Quality Board (2017) Learning from Deaths to identify and communicate common and recurring themes from deaths reported through Datix, across the organisation and to promote embedding of learning from incidents. The post holder will also take responsibility for empowering staff to challenge their own practices and develop new ways of working and work with all NHFT services to make recommendations for practice development and service innovation.
- Provide advice and guidance to other professionals involved in reviewing mortality.
- Promote, support and maintain effective communication.
- Working on own initiative
- Act as an Ambassador for the mortality initiative, overcoming barriers and resolving issues effectively and efficiently.
- Be the named contact for facilitating student placements within the Patient Safety Team.
- Deputise for the Patient Safety Manager during periods of leave or absence.
- Develop and maintain systems and processes.
- Identify issues and share these with the teams.
- Be actively involved in attending and contributing to the Trust Suicide Prevention Group.
- Provide regular update reports (verbal and written) by agreed deadlines.
- Provide detailed reports for Board and Committee meetings in relation to compliance with the national guidance on Learning from Deaths.
- To be the Patient Safety representative for the NHFT Mortality & Morbidity group.
- Work strategically and collaboratively across professional boundaries and Trusts to maximise learning from deaths.
- Ensure that all deaths reported on Datix are ‘channeled’ for review via the correct forum.
- Ensuring that the Learning Lessons Zone section of the NHFT intranet site is updated.
